A worker was killed when he was crushed by the toilet when working beneath the chamber. It lowered during maintenance works and killed him.
Pop up toilets are becoming more common place, the devices were brought into use by Westminster City Council about 20 years ago in an attempt to discourage street urination and are now in many parts of the UK.

A Risk assessment should be carried out to manage the foreseeable risks associated with use and maintenance of the facility.
